8/22/2023 EnchantaVaughn Approved Behavior: Client was observed throwing a fit for no reason, making unnecessary noise, and disrupting her counselor. Client often leaves her therapy area without permission, followed by negative behaviors. Client struggled to refrain from using verbal aggression. The client has to be reminded to communicate positive feedback and not to let her pride get in her way of learning and receiving help. Client was sent to her room to regroup. Client appeared to be moody and exhibiting high discomfort. Behavior: Once client appeared to be calm, she began explaining how she felt targeted by her family. Client did not want to state specifically who but she verbalized things they would say and how it affected her. Client was acceptive of support from her counselor. Positive reinforcement was provided to client during psycho educational. Client promised to be more respectful and listen to her mom and counselor. . Client appeared to be calm after interventions. Although, client continues to need help with achieving ISP objectives. Intervention: QMHP informed client of more acceptable and expected responses to situations that would normally lead client to be triggered and display inappropriate, negative behaviors while in the home. QMHP will review key concepts of being responsible with the family. QMHP will educate client about being respectful, encouraging, mindful, and fair to authority while making good decisions in the home. QMHP will teach client necessary coping skills to decrease impulses. QMHP will provide simple praises and encouragement to continue the positive behaviors she was exhibiting. Intervention: QMHP will brainstorm strategies with client and encourage self-monitoring. QMHP will provide mediation and relaxation techniques to reduce symptoms of ADHD. QMHP reiterated to client the benefits of following the rules and of the privileges she can receive when she does so. QMHP assisted client with identifying privileges that she would like but does not receive due to her negative behaviors; in efforts to stimulate her thinking about cause and effect of her behavior choices QMHP will help client develop healthy self-esteem by reducing level of anxiety and emotional reactions related to home and other independent functioning. Response: Client informed counselor of her behaviors and attitude and how she was working on doing better and taking responsibility for her actions. Client understood the rules in the home but struggled to follow them. Client explained having high expectations made her feel anxious and insecure. Client was not conducting herself appropriately at this time. As the client reflected on her feelings, she stated that she has a hard time controlling her negative thoughts, tone, and words when confronted by an adult. Client plans to work on this daily in order to show progress in her treatment. Response: Client was able to complete psycho social activity with less than 2 prompts to redirect her focus to recognizing triggers and negative reactions to conflict. The client showed her gratitude for being able to release physical tension stating that she feels relaxed and recharged. Client responds well to praise and support. Client understands that her actions have consequences and plans to do better. Client utilized meditation and relaxation techniques when encouraged to do so. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques.
